HRF - I'm guessing one taken with camera behind left shoulder aiming at mirror fired remotely then cloned out; second with camera pointing down arm, then layered onto original. Very clever - well done that man!!!

Your all wrong

Firstly

I mounted a tripod to the sink and took a picture of myself brushing my teeth.

Secondly

I took a shot of the mirror/sink face on and replaced the reflection with the shot I took earlier from the tripod

Thirdly

Snapped a picture of my hand with toothbrush closeup/out of focus and cut it out (removing all background). This was then pasted onto a top layer and wham - bobs your uncle.
