    The exposure indicator should be on all the time in M, never in A or P, but it will appear in S if the 'calculated' aperture is wider than your lens can go - so the shutter speed needs changing. Disclaimer: I could be wrong!

    There's a little tab to lock the aperture ring in place - it's just under where it says '1:1.8D' on the lens barrel. If it isn't locked, you get the error.
