I's only supposed to be good for a couple of stops of stabilisation (=1/160 on a 1.6X crop body) but with care I can get reasonable sharpness down to 1/50. I have VR on my 18-200, but I'm so rubbish at holding everything still I still get movement blur!!
 
I love high ISO...this one was ISO1600 and handheld with an 18-200VR. I didn't have a tripod with me, so to answer the OP's question...yes i would always shoot high ISO and get the shot rather than miss it. The VR helps too.

the noise, but then it depends a great deal on the camera limitations and what you're using/shooting i would have thought?
 
I havent had my D3 for that long but I have been shooting at up to 3200 so far.
Usually around 1600 though.
But It can go up higher just havent saw the need so far for it.
 
as high as my camera goes, crispness and a high shutter speed, and occasionally depth of field are often more important than noise
If I shoot street type photography I will crank the ISO just because I like the noise in the BnW conversion.
 
If I'm shooting portraits I try not to go much higher than 160-250, but when I'm shooting gigs I tend to use 1600. The noise usually adds to the images in those circumstances, and often, black and whites look beautiful with a bit of noise, but it depends on the shot.. gemerally I like to keep it low.
