Difficult this one.

Requirements......

To be of minimum dimensions to contain D90 with 18-55 fitted + divided compartment for second ultra-wide zoom.


No external pouches or protrusions.

Removable shoulder strap.

Lid to lift open, no zips.

Seems very difficult to find anything without pouches on the outside.
 
ahh this sounds like a crumpler job. a cheap crumpler muffin top will do exactly as you want, it has a space for a dslr + lens and another compartment to hold another lens and then theres a small front bit for memory cards/batteries etc.

perfect for your needs.

I LOVE my Crumpler, but the shoulder strap isn't removable, so might not be perfect for your needs?
 
Not exactly what you're after, but if you want simple... one of these might work: LowePro Terraclime

Fixed shoulder strap and single compartment with included lens/camera wrap. Minimal padding.

Available in black, green and, err... plum
 
I haven't come across a combo of all of the above. I have 2 bags, 1 that fits the bill, opens up top, no zips, has compartments, removable strap, but has external pouches (can't remember the make/type of bag). Then I have a lowepro slingshot 200, removable shoulder strap, when you swing it round to your front it effectively opens from the top, has 1 external pouch that is fairly insignificant... but it has a zip!
