I took some pictures recently at my sisters wedding and upon reviewing them some are out of focus and i'm not quite sure why. Shutter speed was good and aperture 5.6

I know I had my flash in slow or rear sync mode by accident but spotted that on the day and fixed it, didn't take many. Is there anything in the exif data that would tell me the flash mode as this would help isolate them.

The only other thing I can think of is I used focus and recompose on most of the oof pictures in question. I've not normally had a problem with this method, but I was using a speedlight on camera in ttl mode and was wondering if this method screws up the ttl? I can't really find any information on this.

Could rear sync do it? Is it possible the subject moved by the time the second curtain went?

I'm not sure, I'm at work at the mo but ill try and get an example up later tonight. Even with the flash firing as it should there are some that are oof and others where the eyes are just not as sharp as others where I didn't focus and recompose.
 
Is the lens calibrated to the body for accurate focus? Has it happened before and can you reproduce the issue with a static subject in controlled conditions?
 
Hi Double i too suffer from this when i think they are perfectly in focus. Have you got examples of the photos. What lens? were you manually or autofocus on them? i find my 1.8g 50mm is out of focus a little on some as the point of focus on the body is about 3" below so i have to compensate! Very annoying when you are trying to do it right.
Usuing TTL shouldn't make a difference i have shot with my speedlight with it although since moving to a d700 rather than my d5000 i notice the speedlight works different. But thats life.

What was the lens? Some suffer from focus creep, in a rush you may not have noticed.
At a wedding I was a guest at, I managed to knock the manual focus knob on the lens as I took it out of the bag (I presume). Took several shots (with the camera confirming focus!), which were miles out, before I realised.

An image (or crop of one) with the exit would be a good help.
 
How much out of focus? An inch? A yard? Is focus sharp on something in front or behind?

If you post a couple of images and the EXIF data we should be able to come up with the reason. A factory reset is often the quickest way to rectify most problems but I would adivicate understanding what the root cause is to, to learn from and it and to prevent it from happening again.
