Another vote for noiseware although pricey at $80 for the photoshop plugin its well worth the money. I found it a bit unstable with larger images until I upgraded my RAM
Noiseware gives you the huge advantage of being able to try several different settings and compare them, all in a single interface (it opens a new tab for each one). It's also much faster than Dfine.
Did (?) Google stop there free download of all the Nik Suite that included DFine? I got it earlier in the year and it works well with LR4. It seems more responsive that the native LR system; maybe LR5 is better but haven't upgraded so far.
