That could be a few reasons, firstly like others have said turn OS off when using a tripod (OS is trying to correct lens shake that isn't present). It could also be miss focus if manually focusing through the viewfinder. Manual focus in love view is much easier especially when zooming in to check. Have you set the viewfinder diopter? Mine is slightly out, manually focusing through my viewfinder is difficult due to my eye sight so I very rarely do it. With landscapes I use live view. LV and long exposures does eat battery power so it's worth getting a spare.

I don't think missed focus as I had lens on infinity.
