ETTR usually has a dramatic effect on reducing noise, by one or two stops. But it is, in effect, also reducing your working ISO by one or two stops...

Agreed. And in some situations that is not possible or desirable.

Yes, ETTR is basically a technique for optimising image quality when conditions are on your side.

When you need to push the ISO, that usually rules out ETTR by definition.
