Apologies if this is too basic a question but what is the best way for setting up for the correct exposure when using ND grad filters and how do you know which is best to use (1 stop, 2 stop etc).
 
Usually a bit of educated guesswork, plus a couple of trial and error shots to get the exposure right.

Not difficult. I just meter as normal, on evaluative, and that's usually pretty close, at least as a starting point. You know where you are then, and can usually go straight to the best strength of grad, with the correct exposure, directly from that one test shot.
 
You can meter for the ground, then the sky, and work out the difference in stops between them.
Or take a test shot and check the histogram. If highlights are still blown then you could use a stronger grad. With experience you get to know roughly what you will need.
