I've just watched a video on youtube which explain that the noise cursor in local adjustment is not actually to add or remove noise as such but can be use to had or remove some of the genereal noise reduction applied to the picture.
So you can use the noise reduction and then with the local brush cancelled that noise reduction over the area of your choice. Seems really great you you have noisy background for exemple.

Seems a very interresting idea.

That is the method I use when I have very noisy backgrounds in my close-up/macro images. As explained in this video I made a couple of years ago I use a slightly different approach, which is quicker than the one shown in the video you linked to. In the video he had Auto Mask turned on for both applying the mask and then removing the edges of the mask that overlapped the background. Instead I have Auto Mask turned off for applying the mask and I apply it with a big brush so it overlaps the subject all round. This is very quick to do. Then holding down the Alt key I then run around the subject to take off the excess. This approach ensures that things like the loose hairs that he missed get included in the mask while still letting you do the operation quickly.

A big advantage of this method which he didn't mention is that it can deal with very strong noise. That is because the global noise reduction is extremely strong, and is much stronger than can be applied with the adjustment brush, so you can deal with stronger noise than you can if you selectively paint noise reduction onto the areas where you want noise reduction. Fortunately, having the adjustment brush noise setting at -100 removes all the noise reduction, no matter how strongly it was applied globally. This means you can get a combination of extremely strong noise reduction on the background coupled with absolutely no noise reduction on the subject.

I sometimes change the adjustment brush setting to a less strong negative setting to brush over midground areas where the noise reduction applied to the background gives too smooth an effect on the midground.
