Following photo was taken with a white background. Silly question maybe, but what have I done wrong as the backgorund is not white.

looking at the catchlights in the eyes it looks like you had at least two heads, did you have a dedicated head for the background? if you did it needs to be set at least a stop higher than your main light
 
It's partially a white balance issue methinks, unless your model really is a light shade of orange :naughty:

If you didn't have the No edit box ticked I could post my quick edit of the shot....

The first answer is more or less right, although I would have said that the light on the background needs to be up to a stop brighter.

Using 2 lights on the face (which is unnecessary and which has made the face look fat) means that you haven't been able to light the background properly. Bear in mind that because the light has to travel further to the background than to the front subject, far less of it has hit the background, that's why it needs to be lit separately with your second light.

I had two head lights and two backlights

still obviously not white, for the reasons Garry outlines above, but whiter....
