How is this effect achieved?

just looking on deviatn art and there are actions to create the effect
 
If I had to guess I'd say it was all lighting, a small low powered light with barn doors to keep it more in the centre of the image??
 
the last photo i waas wondering how this would have been processed, cany anyone help please?

The comment is no flash! So exposure set for the model's face (either manual set by a meter or semi auto and exposure comp). This will over expose the background as you see.

The tone could be from the ambient light or a reflector or as you say possibly processed in Ps.
