How has this been processed?

Increased local contrast enhancement (Clarity in Lightroom, and plenty of it); burning in on creases and wrinkles; a small amount of desaturation of flesh tones; overall exposure value reduced by what looks like a stop or so; chroma noise was removed but lumina noise left alone. It was shot wide open from a distance at full zoom to separate the subject from the background.

It has been a very common 'look' used a lot in magazines. There are probably tutorials out there somewhere but I wouldn't know what might be a good search term. Going over the above steps and playing around will get you close to that look.
 
'High contrast look' might also be worth a google or youtube search ;)
 
It looks to me like 'bleached bypass' preset in colour fx pro by nik software, either that or bumping up clarity in Lightroom to 80-100%, it could also be done in photoshop using high pass filtering, very hard to tell how they did it, hope that helps
