Hi all, wondered if somebody could help me.

other half has taken a few shots at work some include children playing on slides etc with a P+S and would like to use some photos for prints around her workplace. She asked me post and see if theres a generic form which she can print off and take to the parents to get permission for the photos to be used. Iv done a search on her and found one for buildings and one for model release but it seemed to be aimed at models saying they dont mind the images been used rather then the parents ok'ing the childrens photos.

could somebody maybe point me in the right direction or give me any advice.

As long as you have written permission, it should be fine because they have said it's ok.
 
I don't see why you need any permission to do this, at least not from a legal position. However, it is courteous to at least tell people what you are doing as they can get upset about this kind of thing, even if they have no legal right to be.

If you actually want to ask their permission, then a Model Release is the formal way, but a simple form or letter is just as good. What this does is identify the photographer (the Rights holder), the model, and the photographs by way of time and place. It then says that you can do anything you like with them, and is signed by the model or in the case of under 18s, by the parent. A Model Release form will have a couple of lines at the bottom for this.

You can also be more exact about how the photos will be used, and identify that specific use. But this implies that permission is not given for any other future use beyond that.
