Where to buy a new foam insert for an old aluminium squareish suitcase style camera case, any ideas?

Calumet have some at a reasonable price*, but they are more for Peli type cases - you might be able to cut them down though.

have a look at a soft furnishings shop, the sofa foam will be excatly the same so should work!
