D90 + 50mm f1.8 FOCUS PROBLEMS

Hey everyone, been wondering about this for a while so just thought I would start a thread on this as I seen another on a similar problem.
I do wedding photography and sometimes use my 50mm as the Bride and Bridesmaids walk down the aisle.
However I noticed with using this lens and hopped someone could help, as the bride walks down the aisle and I continuously take photos of her walking down alot of my pictures are back focused, the background is perfectly clear yet the people aren't, I know that it isn't my fault as I'm using single point autofocus and it's placed directly on her face at about f2.8 yet it seems to focus on the background :'(
Yet when I have a still subject the lens is perfect.
I maybe get the odd shot in focus but most aren't. Those that are in focus are normally the closer photos as they get within a few meters of me.
Anyone have any ideas?
 
Are you using AFC or AFS? Moving subjects will require AFC but it may be worth using back button focussing for the best of both worlds.
 
I was using AF-C, it was nearly like the d90 foucsed once every few meters rather than constant, but that doesnt explain the background being in focus most of the time. Shall try the back button trick though and see how I find it.

I can't see anything obviously suspect on the Exif. Shutter speed is lowish I suppose but nothing out the ordinary. Maybe worth doing some scientific testing on some static subjects to check focus on that lens.
 
It's almost like the subject has to be a certain size in the viewfinder for it to focus easier? Or maybe it's because the light comming threw the doors behind them threw the camera off a bit?
