There is not any right way to acheive this, it would be easier if you were to post the pic up and im sure someone will come along with help I once read a tutorial on this problem it involved selecting the face putting it on a new layer Ctrl+J then using gaussian blur then lowering the opacity it smooths out most defects then use a soft eraser brush to blend in the edges when I tried it I had great results.
This isnt set in stone I bet theres many other ways to acheive what you want

I know the question originally refered to skin smoothing in CS3, but have you tried Portrait Professional v8.? You can download a fully working trial and it only costs about £39.
I tried 'doing skin' in CS and Corel Photopaint, both are a pain, take a while, and to be honest, if not done correctly just look a bit 'air brushed'. PPv8 is really easy with lost of control and is a programme soley dedicated to doing exactly what you're after.
That Airbrush tutorial tried to shove spyware on my machine!
Luckily I am on a Mac but I'd be wary of that site!
If someone has anti-spyware on their machine and can prove me wrong please do so!

EDIT - Just tried it a couple of times and it's not re-appeared - I'd still click with Caution (unless someone proves me wrong - which will be a good thing!). Looks a good tuturial though!
