Creating a school proof card

I've been asked to do one shot of around 90 kids and 10 adults, with the profits going to the PTA.

Doing the shot itself I'm quite confident with, however I've never needed to create a Proof card with all the details on ie size, prices contact details etc.

So hoping someone can point me in the right direction of how to create the proof card is this done with a template that I can use in LR / Photoshop or is this something I'm going to have to create myself from scratch?

I generally use Loxley for my prints but couldn't find a decent proof in their range.

The sort of template you are looking to design is usually done either in Photoshop or a design program such as In Design, and is usually unique to the photographer who does the school photography. So it is unlikely that you'll find a template on the internet or lab web site. They are not difficult to do but do take some time As it seems to be a fairly limited number of images you won't need to put to much detail on the card. However what does become a bind is adding the class and then the pupils name so that they can be identified by the school. In a schools lab a lot of this information is done automatically by the software. You may need to do this manually. It's not as easy or straightforward as you think. Plus some method of identifying the the correct file to print, possibly simply adding the file name/no to the card
 
Thanks John, that's great advice. Its only one shot ie the whole of Year 2, so only need to do one proof with the fill in boxes so I will look to knock something up in PS.

Im no where near ready to shoot individual school shots, they seem financially rewarding but a lot of effort.
