He then accidentally pulled the trigger, sending a bullet flying through his palm and into the left side of his wife’s abdomen.

I think the article meant 'cleared' the chamber, we normally leave cleaning until after we've finished shooting!

its easier than you'd think, some people set their triggers way too light, drop the gun and the bloody thing goes off.

Modern firearms are drop tested before sale. Although older designs may discharge if dropped modern ones simply don’t do this.
