I use it for most people shots. From portraits to full body, on both my 40d and 5d II. I love the shallow depth of field. I also use for my "Hollywood" portraits.

If you only are interested in the Tonemapping effect and not exposure blending, Topaz Adjust can make Local Contrast enhance the image in that way, or destroy it.
There is also a Tonemapping plugin for Photoshop, from Photomatix.
Play with the sliders in the Channel Mixer to choose which color channel to blow as they will give different results. There are many more colors to choose fron with the Black and White adjustment layer.

Using manual focus is also depenedent on the camera and how big its viewfinder is. To set manual focus on my 350d for instance would be very hard I think, a little less cumbersome on my 40d, an probably ok with a fullframe.
Auto for focus, manual for shutterspeed and exposure.
I change focus points depending on Fstop and where focus should be, epecially when changing from landscape to portrait position.
To focus and then change composition is not a good idea as the focus plane changes .
