The light is coming from in front - gridded / ring flash look ? The tip of the nose, cheeks sort of suggest its fired from in front but firing from the floor upwards rather than downwards. This is reinforced with the arm shadow higher than the arm on the wall behind.

I had both at the same time - with equivalent 70-200 VR lens. I ended up with canon as a personal choice - High ISO and Autofocus was a Nikon win. For me the canon needed a little less processing effort to get an image I loved and my wife was into video so the 5D2 stayed and the D700 was sold.
theres lots of different mods for the quantum - if using with a brolly I use the reflector as a spill kill and bare bulb to get the max light. Lots of choice even a bare bulb enhancer to control light direction on a bare bulb.

I had a 1D IIN and now have a 1DS II. If they are close-ish in price the 1DS is what I would buy every time. The high ISO pixel peeping is about the same and it still does 4fps and up to 32 jpgs in a burst. For me 16mp was a big jump vs 8 and everything else is pretty similar.
The light on the girl is coming from the side. I would say one large light camera left at 90 deg to the girl and then given the amount of light on the boy a large soft box (there isnt much bleed onto the girl (just on her left hand) so I am guessing a large softbox/gridded light set quite high...
