Can anyone let me know the best way to upgrade Lightroom please?

I want to get v5 up and running but also ensure that it picks up all my existing photos and changes that I've made in v4.

Make a backup of your LR4 catalogue before you upgrade. Just in case.

Just follow the instructions. I'd also run a run test integrity and verify as part of the backup process if you don't already. Good news is I've migrated from LR beta ( yes the original one ) to version 5 without any problems . And that includes migrating from PC to Mac
