I'm guessing he used some kind of calibrated dolly where he could move the camera by a specific amount for each shot...

It's mostly done with a Stage Zero dolly, a 6' long pole which you can move the camera base along at incredibly slow speeds to give the grand movements in the video. He also uses another type of dolly for more complicated movements as well

The basic principle is very simple, but getting to his levels is crazy, probably involving bramping, and lots(!) of practice

As above - I've seen this before on the 5D video stuff. Time elapsed photography on a programmed dolly track that moves very slowly from Stage zero.

One of the things I have thought about is doing something similar using Lego NXT as the controller. Very easy to program, put the track on a slight slant to provide some resistance and move the camera along the track by winding a cable/string.
