Earlier this year I got a sigma 120-300mm f/2.8 , its a heavy beast and after my 1st day out with it it had destroyed my already damaged shoulder, so no problem I thought I'll just get a monopod to support the weight and everything will be ok, well I got a monopod and it does indeed take the weight and I can now shoot with the new lens all day, however as I am a big petrol-head and bike nut I do a lot of panning shots and the monopod is not really very good for that as it wants to rotate about either the foot or the head and I want to rotate from my hips which is about half way up the monopod , the solution I devised has worked quite well, I took an old pair of jeans and cut out one of the back pockets including 2 of the belt loops, I place this on my belt at the front and place the foot of the monopod in the pocket so now all the weight is supported on my hips rather than my shoulder and when I rotate my hips the monopod follows .
 
Just had my camera shoulder operated on so know how you feel. Hoping mine gets better soon so I can get back to toggin.
