Printing, profiles and proofing, what gives?

Looking for a bit of advice really, I had a photo printed at Costco over the weekend and before uploading it I made sure that I d/loaded the relevant ICC profile and soft-proofed it in CS5, I've posted below a shot of the JPEG and the end result.

I have no idea what has gone wrong here, my monitor is calibrated via a Spyder 2 Express and as far as I knew I'd taken all the right steps to ensure a consistent print. As it happens it's an end of term gift for my son's teacher so I won't be going to the trouble of getting it printed again but any pointers would be useful to avoid the same happening in the future. :shrug:

so the original is B&W and the costco print has a green hue. if so thats pretty normal if they don't print it as a B&W
