Photographic licence agreement template?

Hey folks,

Does anyone know if there is a template hanging about somewhere on the web for photographic licence rights? A legal agreement with blank fields that I can fill out with whichever model/actor etc so that I may have legal right to use the photos in my own publicity etc. I am not charging for my work but I would still like an agreement all the same just to be on the safe side!
