Tilt adjusts the angle of the plane that is in focus; the standard example being photographing a bookshelf from 45degrees, lens wide open and all the spines in focus, and photographing an acorn on the ground just a few inches away with front to back sharpness (in real life there are difficulties with this ).
You need to examine the image very carefully to check focus is correct, and you nearly always need less tilt than you think.
