Nikon Cashback Question ?

I am trying to sort some cashback from Nikon, I have registered the serial number etc and now the website is asking me to upload a copy of my receipt (which I have) and my Warranty document filled in.

I have the warranty document but apart form the parts circled the form is blank (image of sample form below)

What I wanted to know was do I fill this in myself or should the supplier have filled this in

When I did mine last year for my SB-700, you had to send a jpeg of the receipt, rather than the warranty slip. I've never seen that slip filled in by the camera shop. It's blank apart from the bits that are pre-filled in.

And you know the cashback is not actually cash?
It's a sort of visa card with a credit on it. Great, but you have to remember how much is on it. Maybe creditcardthingback does not have the same ring!

And watch Nikon. Although I filled all the bits in and jpeg'd the receipt, they still needed prompting to send the "cashback" card out. They were very slow................

Thank you - yeah I have just phoned them and they said the same, I filled the card in myself and yes I did read it was a card with money on but it can be spent anywhere a Visa is accepted so just as good really
