Model Release Forms

Evening all,

Has anyone got the above forms they could share? I am looking for one for over 18's and one for under 18's. Have some portraits which have been accepted for editorial (book) and they require the above before I can submit/get paid.

Wanted to revive this thread to ask a question thats been bugging me about model releases. Its not something I am using at the mo, but you never know in the future.

What happens about model release forms if you are dealing with clients purely by electronic means? I mean, I pressume some images are sold through the interent, emails etc, say to clients abroad, etc, so how can you pass on a release form thats signed? Is it a case of even in electronic cases, get a snail mail address and use it? Sorry, just another of those issues that never having sold any images, I have never had to deal with, so just wondered what the etiquette is for these things? :shrug:
