Metering issues with R5 and RF 70-200mm f2.8

I have recently experienced some metering issues whilst photographing a local 10K run. I have my camera set to Manual, aperture set to f5.6, Shutter speed set to 1/500 sec, and ISO set to Auto. I was using eye-tracking focus in Servo Mode, Evaluative Metering and back button focus.

The majority of my photographs were fine with the exposure indicator staying at 0 when taking the photographs.
However, every so often the exposure meter indicator would shoot to the left, the iso would go to the maximum I’ve set for Auto ISO of 12,800. Needless to say the image came out almost black. This seemed to happen when I attempted to zoom whilst holding the back button focus button, but I cannot recreate it on a regular basis.

I’ve wracked my brain as to the possible cause of this but have not found an answer.

Any suggestions would be welcome.
