Like all photography it's more about being in the right place at the right time, the gear helps but if you aren't there you won't get the shot!
 
Fascinating reading and boy do these guys and gals earn their money!

I wonder if there comes a time in all their careers when they simply can't "hack it" anymore due to the stress involved because I cannot see a picture editor accepting excuses.

Just out of interest and if anyone knows, are these top agency photographers a result of good degree's/courses or are there many that are self taught?

Same as anywhere else, some will have studied photography "formally", some will have spent time carrying bags and making tea, and some will have just started shooting.
 
But how do you end up on the books for the likes of Getty and AP?

Start knocking on doors I assume. It's not something that interests me, but it didn't take long for a mate to start work for Getty / Lonely Planet and a few other agencies once he decided that was what he wanted to do. I don't think they're like some kind of mysterious club, they're just looking for people who can produce work they can sell - like everyone else.
