How is this achieved?

I would guess she has had a digital makeover to remove any flaws in her skin then had a finish added to give the colour/shine

I did something which maybe similar in a much milder way as it was a desaturated image to the photo in post 11 in this thread

How is it achieved - with hours (many hours) of practice plus quite a few working on the final image :D
 
perfect imaging has got to be heavily photoshoped and also heavy on the make up, which would also hide blemishes etc- mega money spent either way one would imagine.
