How do I recreate baby B&W

Hiya folks

My Little sister has just had a baby and I would love to recreate something like the shot below.

How,what do I need.

Well the black background was created from cutting the image out.. this is most obvious on the left on the baby's arm and under the hands at the bottom.

There was a light source on the left, you either want off-camera flash with modifier (for softer light) or to bounce light off a wall on that side.

I've got an Sb900 so that'll do for off camera. (about time it actually got used).

I'll get her to wear something black too that may help a little.

If you light it properly, there shouldn't be a need for a cut-out. Just keep the background as far back as possible and the light-source close (front-above?). You'd need a small softbox though.
 
not withstanding the lighting
i really cant see the baby's face
the hands give the shot, and quite progressive it is, a disturbing effect, almost like a deformity
