How do I create a desk top icon?

I want to create a desktop icon. CS6 doesn't allow me to save as .ico and googling 'tinterweb just points me to anyone of a number of downloadable programmes that may or may not be kosher.

Can anyone recommend either a good free programme, or some other method of doing this?
