HELP My photos are grainy

Help- I dont know what i've done.
I have a NIKON D40X, I use it manually, for the past few days, all the pics i've taken look grainy or out of focus.
I'm using the kit lens 18-55mm, using the auto focus.
Shots i've taken with the built in flash or my SB600 dont seem quite so bad.
But before my photos were sharp & now they are not.

I may have done something stupid, is there anything obvious that i could have done?
The following pic was taken at ISO 200, shutter 1/200, F11, focal length of 39mm.
I've cropped it close so hopefully the problem will show on here.

help appreciated,
oh & did i mention, i'm doing a shoot tomorrow at 10am !!-

Yes, i've got lots to choose from, will try to make it bigger

Do a two-button reset. Hold down the two buttons with the little green dots next to the info button and the magnifying glass with + sign for more than 2 secs. This will change the camera to default settings but wont affect your custom settings. Set your camera to how you want it, and then try reshooting. Did you have your ISO set on auto, if you did it probably auto selected a very high ISO 1600 or 3200 HI even.
 
Both pics were taken manually at ISO 200.
The little girl has the same blur & was taken yesterday in sunlight & the lady in lowlight with a flash. I'm looking thru viewfinder, waiting for it to auto focus- it beeps & a little dot shows on the viewfinder, so how can it still be out of focus?

i'm no great tog, but my pics dont usually look like this.
Going to try the reset now

Now if my photos are rubbish i'll have to come up with another excuse!

Yes I guessed it would work - I've had to do it myself a few times.
