Has anyone got a spare 100 - 400L rebate voucher??!?!

I stupidly put my 100 - 400L rebate voucher somewhere safe, to safe, as I now cannot find it which is annoying. Whats annoying is that I have prepared my paper work to send to canon. Its somewhere but I simply cannot find it.

So, anyone happen to have a spare copy knocking around that I can pinch so I can send it off to Canon?
