Currently I have my Flickr account Privacy setting to exclude downloading of my images.
I see that there are various levels of Privacy including who can download images, e.g. Friends & Family.
  • Question 1. ... how do I assign someone as Friends & Family?
  • Question 2. ... will this absolutely limit permission to this person?

There is an extensive help section on Flickr. Did you read through there?
 
What he said. Flickr regularly change things and their help files are kept up to date.

It's changed since I last set up my Friends list, starting from your Flickr homepage

Note, Friends and Family must have a Flickr account and use that to view your stream for the permissions you set to take effect. You may also need to set permissions in Organisr depending what you want to achieve.
