If I combine a 0.9 soft grad with a 10 stop ND do I then need to calculate the exposure time for 13 stops?
 
No, because you'd end up cancelling the effect of the grad that you want and blowing out the foreground. Just calculate for the 10 stopper.
 
When I used filters, I always followed the camera's meter and didn't need to calculate anything. Always thought I was doing something wrong but couldn't figure out what - results seemed OK!
