Yes as mr pants suggests i would duplicate the layer, blur it to taste, apply a reveal all layer mask (all black), and use the circle gradient tool set to white to dragging out from the middle. You can adjust the strength of a layer mask to taste by using the levels.
blimey i must admit, im scared of 1 or 2 of those examples, she seems to have taken on a maniacal look with grey skin.
Im quite a big fan of sodding the subtle and completely cleaning up every mark blemish or shadow.

You dont have to shoot for it, it can be done in PS.
Simplest and quickest method is as followed.
Desaturate you photo.
Open levels (ctrl-l)
Move the white and black slider in to raise the contrast of the photo, untill you have a normally exceptable black and white.
Duplicate the layer...
In the filters menu is a filter at the top called liquify. It loads up your picture and allows you to nudge areas around according to brush size, density and pressure. Its the perfect thing for the task.
Once you have the shape, use the patch tool, and clone tool to remove wrinkles and lines...
I nicked it from a tutorial i had done some while ago, it was about using apply image in PS to do just what i did. Was a bit mish mashed as it hasnt blended perfectly. :(
The hands where cut from there background via colour range and blended in using layermasks and blend modes.
You can go about this 2 or 3 ways
In PS CS2 you have two tools that would do a good job.
Select the clone tool and changing the mode to lighten, select a fair bit of skin and paint over the other (this lightens the redness)
Or
Select the heal brush, and do as above
Or click the [o] button...
